When she's close to the player she will start quickly opening and closing all the doors close to her, when this happens walk to a direction locking back and you'll see she appearing. She spawns in the exact location of the player but if you don't pay attention she might just spawn without you seeing her. (the only way to evade her attack is dodging right before she attacks).
