I've got a couple of ideas:

A second hex-region adjacent to the official one.
A collection of different groups and factions.
An expanded spell list.
A set of locations that changes tiers, depending on different circumstances.

I love the idea of getting some faction play into the game! Plus another region would mean I could play without knowing all the secrets ahead of time lol
